How is each field of the scanned data transferred to Leads/Contacts/Accounts?

Scanned card data are transferred as shown below.
You can customize destination fields at Field settings. See here for more information.

*If the Automatically validate address function in the mobile app is turned off, label information for numbers 8–12 will be reflected under "Street".
*If the Separate Department and Title function in the mobile app is turned off, Lead label information for numbers 3-4 will be reflected under "Title".
*The uploader is the default owner unless a certain owner is assigned by checking the Set owner user for Lead/Contact box.
